When to use this implant template
Use it for surgical placement of an endosteal implant (D6010), with or without grafting at the same visit. An implant note is a surgical record and also a product record. Years later, a restorative dentist or a manufacturer warranty claim will need the system, size, and lot number, and insertion torque explains why you loaded the implant when you did.
What an implant note must include
- The site and the imaging used for planning.
- Risk factors reviewed, such as diabetes, tobacco, and antiresorptive drugs.
- Implant system, diameter, length, and lot number.
- Insertion torque or ISQ, which supports your loading decision.
- Grafting, closure, and sutures, plus post-op imaging.
- Post-op instructions and prescriptions, and when you plan to restore.
