When to use this denture template
Use it at delivery of a complete or partial denture and at each adjustment visit after. Denture patients often return several times in the first weeks. A short, consistent note each time shows the history of the fit: what hurt, where, what you adjusted, and that the patient approved the look before leaving.
What to document
- The prosthesis and arch, and whether this is delivery or an adjustment.
- Fit checks: retention, stability, borders, and pressure-indicating paste.
- Occlusion and how it was verified.
- Patient approval of esthetics and phonetics, which matters if they later ask for a remake.
- Every sore spot and adjustment, by location.
- Home care instructions, including removing the denture at night.
Example adjustment note
Patient seen for post-delivery adjustment of a mandibular complete denture. Patient reports: sore spot, lower left lingual. Inspection: intaglio and borders smooth. Fit: pressure-indicating paste used, retention adequate. Occlusion: even bilateral contacts verified. Soft tissue: sore spot/ulceration noted, mandibular left lingual flange area. Adjustments: pressure spots relieved, polished after adjustment. Instructions: remove at night to rest the tissue. Follow-up: 1 week.
